Below are general guidelines to consider when determining your minimum stay in town, please keep in mind that these can change depending on your own personal healing process:

Instructions for Out of Town Patients
We recommend that you call our office to find out about the availability of specific dates for your consultation or procedure.
For patients interested in scheduling the consultation and procedure in the same week, we request that you fill out the New Patient Check-in Forms linked below and return that to the office at info@sighttrust.com. Please make us aware of any known medical conditions that may require a medical clearance prior to surgery. More information can be found under the guidelines for possible medical clearance section.
Once the New Patient Check-in Forms have been reviewed, our staff will set up a personal phone consultation to discuss your surgery in detail. If for any reason you are unable to return the requested History and Physical form, please let us know. A member of our staff would be happy to review your History and Physician information verbally over the phone.
The following are guidelines for possible medical clearance requirement:
- Severe or known Anesthetic issues in the past
- Chronic Obstructive Pulmonary Disease (COPD) – Sleep Apnea and/or CPAP use.
- Congestive Heart Failure (CHF)
- Kidney Failure / Organ Transplants
- Stroke/ Seizures/ Aneurysm
- Severe Neurological Disorders/ Paraplegia
- Arrhythmias (AFIB)/ Heart Attack (MI)/ Heart Disease/ Open Heart Surgery
- Advanced Age (80+)
- Severe Obesity (BMI 35 +)
- Multiple Cancers or Transplants
If you have any of the above diagnoses, please make a member of our staff aware. They will assist you with the necessary Medical Clearance Form and provide those to your primary care physician and/ or cardiologist as needed. Medical clearances are required to be dated within 30 days of your scheduled surgery and are accepted for 30 days thereafter should you need to postpone for any reason.
