Welcome to SightTrust Eye Institute! In order to help make your upcoming appointment with us a breeze, we recommend that you click on the first link below (New Patient Check In Forms), print and fill out the forms, and bring them with you to your visit.
If for any reason you are unable to fill out your paperwork in advance, please arrive at our office 20 minutes before your appointment so that you can take your time to complete the forms when you get here. Remember to bring your driver’s license and insurance cards as well – we will need to make copies of them before we get started.
